Lexmark C736dn is printing a ID job page in every job

My Lexmark C736dn is printing a ID job page in every job, i search in configuration menu an option to desactive it but i didn't find, it happens only when i send a job from the computer to the printer but when i print something like a report of the own printer that doesn't happens. I use two OS: Windows 10 and Lubuntu 15.04 and both happens. I tried to reset to default configs and doesn't works too.

I fix the problem. Access the printer menu by typing your ip in a browser then Settings> General Settings> General Network Settings (Active Card) and uncheck the Cover Page option.
