Is this a broken printhead?

Hi

When printing (and this photo shows a 'test page') - I just get black and grey boxes all over the page like this:



Any ideas if this is caused by a broken printhead, or whether it is something else?

Printer is Lexmark Pro901 - have tried all self repair i.e. deep clean, align etc. All comes out the same.

it seems like your printer head is damaged that's why the sample print page which you have posted here is having blurred ink, if your printer is under warranty get it checked from the reseller where you bought it from, or if warranty is expired, get it repaired from some third party technician.
