Poor ink transfer on foamex?

Hi guys,

Would anyone know what causes this when printing on a FoamEx sort of material, 3mm thick? It was done by a local printer here.

Is it the design file supplied (which was print .pdf) or the printer? not sure what went wrong =/

its about an a1 board. find attached the image close up of the ink running in the text.
